Stuttering assessment and treatment: from preschool to adulthood
In the preschool years treatment goes through the parents — guidance, interaction therapy (Palin PCI) or the Lidcombe Program — without turning the child's attention to his stuttering; for older children and adults the choice is between fluency shaping and stuttering modification, with work on feelings and thoughts added.
Why it matters in the exam
Questions on stuttering treatment test principles: treatment can start close to onset; a 3-year-old is not made aware of his stuttering; at two and a half the accepted recommendation is parent guidance; and in the first assessment of a preschool child the central question is whether this is developmental disfluency or stuttering. The length of treatment is not predicted at the first session.
For adults the questions compare approaches: fluency shaping works on speech as a whole — slow rate, easy onset and light contact — and does not address emotional attitudes; stuttering modification (Van Riper) deals with coping with the stuttering moment itself and with desensitisation, that is, getting used to speaking in anxiety-provoking situations. In cognitive behavioural therapy extreme thoughts are replaced with accurate ones, and internal versus external locus of control is asked too. The method is chosen for each person individually, and the therapeutic alliance is a central factor in success.
Key points
- Preschool: through the parents — guidance, Palin PCI, Lidcombe.
- Fluency shaping: all of speech, slow rate, no emotional work.
- Stuttering modification: the stuttering moment and desensitisation (Van Riper).
- CBT: accurate thoughts instead of extreme ones; internal locus of control.
- The SSI measures frequency, duration and concomitants; questionnaires measure attitudes.
Summary table
| Age or approach | Principle | Tools | Exam point |
|---|---|---|---|
| Preschool — indirect guidance | Change the environment without drawing attention to stuttering | Regular routine, turn-taking in conversation, relaxed parental rate | A young child is not made aware of his stuttering |
| Palin PCI | Equip parents to change how they interact with the child | Watching the interaction, "special time" at home to practise the chosen behaviours | Watching the interaction helps diagnosis and planning |
| Lidcombe | Parental verbal responses to fluent speech and to stuttering | Praise for smooth speech and a gentle request to correct, daily measurement | A direct treatment delivered by the parent |
| Fluency shaping | Change speech as a whole into fluent speech | Slow rate, easy onset, light contact, continuous phonation | Does not address emotional attitudes |
| Stuttering modification (Van Riper) | Cope with the stuttering moment and reduce fear | Identification, desensitisation, cancellation, pull-out, preparatory set | An adolescent: first bring him closer to his stuttering |
| CBT and emotional work | Change the thoughts that lead to feelings and behaviour | Cognitive restructuring, attitude questionnaires, SSI for severity | "It is unpleasant when I stutter" instead of "it will be a disaster" |
