Cardiac CT and MRI

Coronary CT angiography excludes coronary disease at low-to-intermediate probability, and the calcium score stratifies risk; cardiac MRI measures function and characterises tissue — and the late gadolinium enhancement (LGE) pattern separates infarction (subendocardial) from myocarditis (subepicardial) and cardiomyopathies.

Why it matters in the exam

CCTA suits stable chest pain at low to intermediate probability, emergency chest pain at low-intermediate risk, coronary anomalies and graft assessment; it suits less a heavy calcium load, arrhythmia and a fast heart rate (target below 60–65 with a beta-blocker). The calcium score (Agatston) is done without contrast: for each lesion area is multiplied by a density factor according to peak HU (130 and above) and summed; 0 — very low risk, 400 and above — severe. High-risk plaque: positive remodelling, low-attenuation plaque, spotty calcification and the "napkin-ring" sign. Coronary aneurysm: dilatation of 1.5 times or more the adjacent normal segment.

Anatomy: dominance is defined by the artery giving the PDA; in most people (about 80%) the right artery is dominant, and in left dominance the LCx gives the PDA. Malignant anomaly: a coronary artery arising from the opposite sinus and running between the aorta and pulmonary artery (interarterial) — associated with sudden death in young athletes. Coarctation: rib notching of ribs 3–8, the "3" sign, and association with a bicuspid aortic valve and Turner syndrome. Before TAVI, CT measures the annulus and access arteries; before atrial fibrillation ablation CT shows the pulmonary veins (usually 4) and excludes appendage thrombus.

MRI: cine is the reference method for EF and volumes. LGE: in infarction — subendocardial to transmural in a coronary distribution; a dark core within the enhancement is microvascular obstruction (MVO) and a poor prognostic sign. In myocarditis — subepicardial or mid-wall, usually inferolateral, with oedema on T2 (Lake Louise criteria: one T2 criterion and one T1 criterion). HCM — patchy mid-wall at the right ventricular insertion points; amyloidosis — diffuse subendocardial with difficulty "nulling" the myocardium; Takotsubo — apical ballooning with oedema and no significant LGE. Native T1 is high in oedema, fibrosis and amyloid, and low in Fabry disease and iron overload. To distinguish thrombus from tumour — thrombus does not enhance on LGE with a long TI. Lipoma — fat signal that disappears with fat suppression. The commonest primary cardiac tumour in adults is a left atrial myxoma; metastases are far more common than primary tumours.
