Scrotal pain and masses

Sudden severe testicular pain in an adolescent is torsion until proven otherwise and needs immediate surgery; a hard painless testicular lump is a tumour until proven otherwise and needs an urgent ultrasound — and the other causes (epididymitis, hydrocele, varicocele) are diagnosed only after these are excluded.

Why it matters in the exam

The exam presents a 13-year-old with 4 hours of testicular pain and vomiting, a 21-year-old with a few hours of pain, a 30-year-old with a month of discomfort and swelling, and a question asking which listed factor does not raise testicular cancer risk. It asks for the next step.

Time is the decisive factor in torsion: the chance of saving the testis is very high in the first six hours and falls quickly afterwards — so with high suspicion you do not wait for ultrasound.

The family doctor is usually the first to see the boy or the man, so the quick decision — who goes straight to theatre, who has an ultrasound that week and who is treated with antibiotics — is the whole point. Inguinal swelling also appears in questions.
