Ankle sprain and foot pain

In an ankle sprain the Ottawa rules decide who needs an X-ray, and most sprains are treated with early movement, not a cast; in the foot the site of the pain almost decides the diagnosis.

Why it matters in the exam

Ottawa ankle-rule questions return in almost every paper ("which of these patients should have an X-ray?"). Beside them appear heel pain on getting out of bed (plantar fasciitis), burning pain between the toes (Morton's neuroma), a runner with point bone tenderness (stress fracture), and a child with heel pain on activity (Sever's disease).

Remember the Achilles tendon rupture — a "kick from behind" while running and a positive Thompson test — as a diagnosis that must not be missed.

The Ottawa rules were developed to reduce unnecessary X-rays without missing fractures, and their sensitivity is close to 100%. So a patient who meets no criterion does not need an X-ray, even if the ankle is very swollen.

Foot pain in an older adult with diabetes also requires checking sensation, pulses and skin, because a small wound can become a deep infection. A stress fracture in a female athlete should raise questions about nutrition and menstruation.
