Anaphylaxis and angioedema in the emergency department

Anaphylaxis is a life-threatening systemic allergic reaction, and the first and only life-saving treatment is intramuscular adrenaline — early and without hesitation; angioedema without urticaria suggests a bradykinin mechanism, in which adrenaline and antihistamines hardly help.

Why it matters in the exam

Exam questions test three things: recognising anaphylaxis even without a rash (hypotension or bronchospasm straight after exposure to a known allergen), choosing the right drug, dose and route — adrenaline 0.5 mg into the thigh muscle in an adult — and understanding that antihistamines and steroids are not first-line treatment.

The second group of questions deals with angioedema: a patient on an ACE inhibitor with lip and tongue swelling, or a young person with recurrent attacks, abdominal pain and a low C4 (hereditary angioedema). Here the answer is airway protection and a specific drug — icatibant or C1-INH concentrate — not repeated adrenaline.

Know the aftercare too: after stabilisation, patients who needed more than one dose of adrenaline or had a severe reaction are observed for longer because of possible biphasic reactions, and they leave with two auto-injectors, practical teaching, a written action plan and an allergy referral.
