Mechanical heart valves need lifelong warfarin (target INR about 2.5–3.5 for the mitral position), with low-dose aspirin if bleeding risk allows. DOACs such as apixaban and dabigatran are contraindicated with mechanical valves because of excess thrombosis (RE-ALIGN, PROACT Xa). Antiplatelet agents alone (א, ב) are inadequate.
