Psychosocial ageing, loneliness and bereavement

Most older adults keep good mental well-being, but losses (a spouse, health, a role and independence), loneliness and social isolation are common and as harmful to health as "medical" risk factors; normal grief, prolonged grief (a disorder) and depression must be distinguished — and each is managed differently.

Why it matters in the exam

The exam presents a widow who lost her husband two months ago, cries and misses him but continues to function and enjoys her grandchildren — normal grief, supported without drugs. By contrast, if a year has passed and she is still absorbed in longing, cannot accept the death and does not function — prolonged grief disorder (DSM-5-TR, adults — at least 12 months). And if there is a general sense of worthlessness, guilt unrelated to the deceased or suicidal thoughts — depression.

Recurring points: loneliness (a subjective feeling) and social isolation (few contacts) are linked to increased mortality, dementia, depression and heart disease; hearing loss contributes to isolation; older men, especially widowers, are at the highest risk of suicide; and retirement, moving home and a spouse's illness are transitions that raise risk. Theories that are asked about: Erikson's "integrity versus despair", and "successful ageing" — health, function and social engagement.

Candidates are also asked about the "paradox of well-being in old age" — despite losses and illness, many older adults report high life satisfaction — and about the main caregiver, usually an older spouse or a daughter, who is at risk of burnout, depression and illness.

Supporting the caregiver is also part of caring for the older adult.
