Long-term care in Israel: functional status and care settings
In Israel an older person's functional status — independent, frail, nursing, mentally frail or complex nursing — determines which setting suits them and who supervises it; in the community the National Insurance Institute pays a long-term care benefit according to a dependency assessment, and most dependent older adults are cared for at home.
Why it matters in the exam
The exam describes an older adult and asks for their status: one who needs help with some daily activities but is mostly mobile — "frail"; one who is bed-bound, in a wheelchair or dependent on help in most activities — "nursing"; one whose main problem is cognitive impairment requiring supervision — "mentally frail"; and one who needs close medical and nursing care (for example severe pressure injuries, parenteral nutrition, ventilation) — "complex nursing".
Recurring points: frail-care wards in homes for the aged are supervised by the Ministry of Welfare, and nursing and mentally frail wards by the Ministry of Health; complex nursing care and geriatric rehabilitation are provided through the health funds as part of the health basket; and the National Insurance long-term care benefit is given to people living in the community, according to their dependency in daily activities and need for supervision.
Most dependent older adults in Israel live in the community and are cared for by family members and paid caregivers, often foreign workers, and the burden on the family is high. The exam also asks about day centres, sheltered housing and the role of social work in coordinating services and submitting applications.
The decision about a setting is always made together with the patient and family.
Key points
- Five statuses: independent, frail, nursing, mentally frail, complex nursing.
- Frail — Welfare; nursing and mentally frail — Health.
- Complex nursing and geriatric rehabilitation — through the health funds.
- In the community: National Insurance long-term care benefit by dependency assessment.
- Rehabilitation comes before a decision on a permanent setting.
