Forensic neuropathology of head injury
Head injury is interpreted by the type of damage — skull fracture, epidural or subdural haemorrhage, contusions at and opposite the impact, and diffuse axonal injury — and each finding points to a different mechanism: direct contact, acceleration, or a moving head falling onto a fixed surface.
Why it matters in the exam
The exam uses this topic to ask about mechanism. In a fall, the moving head stops suddenly on a surface, and the brain contusion opposite the impact (contrecoup), usually at the frontal and temporal poles, is larger than the contusion at the impact site (coup). In a blow with an object to a stationary head, the coup contusion predominates. Epidural haemorrhage usually results from a temporal fracture tearing the middle meningeal artery, with a possible lucid interval and then rapid deterioration. Subdural haemorrhage results from tearing of bridging veins in acceleration-deceleration and is common in older adults, people with alcohol use disorder and infants.
Traumatic subarachnoid haemorrhage at the base of the brain after a blow to the side of the neck or the jaw, sometimes in a fight between young men, can result from a tear of the vertebral artery, and it must be sought by targeted dissection of the neck arteries. Basal skull fractures show direction: a ring fracture around the foramen magnum fits a fall onto the feet or the vertex, and a transverse "hinge" fracture across the base fits a broad blow to the side of the head, for example in a motorcyclist.
Diffuse axonal injury (DAI) is caused by rotational forces and shows as small haemorrhages in the corpus callosum and brainstem, and microscopically as axonal swellings that stain with β-APP a few hours after injury. Brain swelling and raised intracranial pressure lead to herniation: subfalcine, uncal herniation through the tentorium with pressure on the third nerve, and tonsillar herniation through the foramen magnum. Pontine (Duret) haemorrhages result from transtentorial herniation stretching the paramedian arteries — a secondary finding of pressure, not a direct blow to the brainstem.
Key points
- Fall: the contrecoup contusion is larger.
- Epidural: temporal fracture and artery; subdural: bridging veins.
- Blow to the neck with basal subarachnoid bleeding: vertebral artery.
- Axonal injury: rotational forces; β-APP.
- Duret: pontine haemorrhages secondary to herniation.
Summary table
| Finding | Mechanism | Feature | Interpretation |
|---|---|---|---|
| Epidural haemorrhage | Temporal fracture, middle meningeal artery | Biconvex; possible lucid interval | Direct blow |
| Subdural haemorrhage | Tearing of bridging veins in acceleration | Crescent; older adults, alcohol, infants | Even with moderate force |
| Contrecoup contusion | Moving head stops on a surface | Frontal and temporal poles | Fall |
| Diffuse axonal injury | Rotational forces | Corpus callosum and brainstem; β-APP | Strong acceleration |
| Duret haemorrhages | Transtentorial herniation | Pontine haemorrhages | Secondary to pressure |

(the question as examined, in Hebrew)
(the question as examined, in Hebrew)
