Drug-related deaths — autopsy findings and the role of the forensic pathologist
In drug deaths the autopsy findings are usually non-specific, because the substance acts pharmacologically and does not produce a structural lesion; the forensic pathologist's role is therefore to exclude other causes, identify signs of use and complications, and combine toxicology with circumstances to decide cause and manner.
Why it matters in the exam
Questions ask what is found, why almost nothing is found, and what the complications of injecting are. In opioid death the typical finding is heavy pulmonary oedema with froth in the airways and sometimes a "froth cone" at the mouth, with fresh and old needle marks; the risk of death is especially high after a period of abstinence — release from prison or detoxification — when tolerance has fallen. Methadone is eliminated slowly and so accumulates in the first days of treatment or in a non-tolerant person, and death may occur hours after the dose.
With stimulants (cocaine, amphetamines) death results from arrhythmia, myocardial infarction and cerebral haemorrhage, from hyperthermia with rhabdomyolysis, and from "excited delirium" during restraint. With MDMA at parties there is a danger of hyperthermia and of hyponatraemia from drinking a lot of water. Inhaling volatile solvents causes sudden death from arrhythmia when the heart is sensitised to adrenaline, sometimes with a rash around the mouth; there are no special autopsy findings. Some drugs and medicines cause acute liver failure — mainly paracetamol and MDMA.
Complications of injecting: skin and lung abscesses, tricuspid-valve endocarditis, septic pulmonary emboli, hepatitis C and HIV, and "foreign-body granulomas" in the lungs from fillers of crushed tablets, seen as birefringent crystals under polarised light. Swallowing drug packets for smuggling (body packing) causes bowel obstruction or acute poisoning if a packet opens, and imaging locates them. The manner of death — accident or suicide — is decided by circumstances, a note, history and the pattern of findings, not by the concentration alone.
Key points
- Findings are non-specific; the role: exclude, identify complications and integrate toxicology.
- Opioids: pulmonary oedema, froth, needle marks; risk after abstinence.
- Stimulants: arrhythmia, infarction, haemorrhage, hyperthermia.
- Volatile solvents: sudden death with no finding.
- Injecting: right-sided endocarditis, abscesses and foreign-body granulomas.
Summary table
| Group | Autopsy findings | Special risk | Pitfall |
|---|---|---|---|
| Opioids | Pulmonary oedema, froth cone, needle marks | After abstinence; methadone accumulates | Expecting a structural lesion |
| Stimulants | Cardiac enlargement, infarction, cerebral haemorrhage, rhabdomyolysis | Hyperthermia, delirium in restraint | Ignoring the restraint |
| MDMA at a party | Non-specific | Hyperthermia, hyponatraemia, liver | Thinking only of overdose |
| Volatile solvents | No special finding; perioral rash | Sudden arrhythmia | Searching for a lesion at autopsy |
| Injecting | Abscesses, right-sided endocarditis, foreign-body granulomas | Infections and emboli | Forgetting to examine lungs under polarised light |
