Cataract and cataract surgery
Cataract is opacity of the lens that causes a gradual, painless decline in vision; the decision to operate depends on functional impairment rather than on a specific visual acuity number, and the standard operation is phacoemulsification with an intraocular lens implanted in the capsular bag.
Why it matters in the exam
In the exam three types are distinguished: nuclear cataract — typical of older age, causing a myopic shift ("second sight" — the patient reads again without glasses); cortical cataract — spokes from the periphery, glare from lights; posterior subcapsular cataract — in young people, after steroids, in diabetes and after radiation, with glare and reading difficulty early on. The lens produces most of its energy (ATP) by anaerobic glycolysis, because it has no blood vessels.
Systemic links that are asked: diabetes ("snowflake" cataract in young poorly controlled patients), myotonic dystrophy (coloured "Christmas tree" cataract), Wilson disease ("sunflower" cataract), galactosaemia ("oil droplet" in the newborn), atopy (shield-shaped anterior subcapsular cataract), electrical injury and radiation. Before surgery biometry is done — axial length and corneal power — and a formula is chosen to calculate the lens.
Complications are the core of the questions. Early: posterior capsule rupture with vitreous loss; TASS — sterile inflammation within 12–24 hours with limbus-to-limbus corneal oedema; endophthalmitis — 2–7 days later, pain, reduced vision, hypopyon and vitreous haze (prevention: povidone-iodine 5% and intracameral cefuroxime). Late: cystoid macular oedema (Irvine-Gass) after 4–12 weeks; posterior capsule opacification — the commonest late complication, treated with Nd:YAG capsulotomy; retinal detachment, mainly in high myopes. Tamsulosin causes IFIS — a floppy billowing iris and a constricting pupil — even after the drug is stopped, so the surgeon must be told.
Key points
- Lens opacity with gradual, painless loss of vision.
- Nuclear → myopic shift; posterior subcapsular → steroids and diabetes.
- Operate on functional impairment: phaco and an in-the-bag lens.
- Endophthalmitis 2–7 days; TASS 12–24 hours.
- Commonest late complication: posterior capsule opacification → YAG.
Summary table
| Complication | When | Signs | Treatment |
|---|---|---|---|
| TASS | 12–24 hours | Limbus-to-limbus corneal oedema, little pain | Frequent topical steroids |
| Endophthalmitis | 2–7 days | Pain, hypopyon, vitreous haze | Vitreous sampling and intravitreal antibiotics |
| Irvine-Gass (CME) | 4–12 weeks | Reduced vision, cysts on OCT | NSAID and steroid drops |
| Posterior capsule opacification | Months to years | Gradual blur, glare | Nd:YAG capsulotomy |
| Retinal detachment | Months to years | Flashes, floaters, a curtain | Urgent retinal surgery |
