Bariatric surgery and its complications

Bariatric surgery is the most effective treatment for severe obesity and its associated type 2 diabetes; the indication is set by BMI and comorbidities, and the complications — leak, internal hernia, marginal ulcer and nutritional deficiencies — are what the exam asks about.

Why it matters in the exam

Under current guidelines, surgery is recommended from a BMI of 35 regardless of comorbidity, and considered from a BMI of 30 when there is metabolic disease, for example type 2 diabetes that is poorly controlled (thresholds are lower in Asian populations). The most common operation is the sleeve gastrectomy, which can worsen reflux; a Roux-en-Y gastric bypass is preferred for severe reflux or diabetes but carries more nutritional deficiencies.

Clinical questions focus on complications: a heart rate above 120 in the first days is a leak until proven otherwise, even when the abdomen looks quiet; recurrent abdominal pain after gastric bypass, even with a normal CT, needs laparoscopy for a suspected internal hernia; a marginal ulcer is linked to smoking and NSAIDs; and gallstones are common with rapid weight loss. In the long term iron, B12, calcium, vitamin D and thiamine must be monitored, and pregnancy should be delayed for a year to a year and a half after surgery.
