Common bile duct stones and acute cholangitis

A stone blocking the common bile duct turns the biliary tree into a closed, infected reservoir; in cholangitis antibiotics alone are not enough — the bile ducts must be drained, usually by ERCP.

Why it matters in the exam

Questions on this topic turn on two decisions. The first is diagnostic: a patient with gallstones and abnormal liver tests — is the probability of a duct stone high enough to go straight to ERCP, should MRCP or EUS come first, or can the patient go to surgery without further work-up? The answer depends on a risk grading built on the ultrasound finding, the bilirubin value, the duct diameter, age and the presence of cholangitis.

The second decision is therapeutic and urgent: a patient with fever, jaundice and right upper quadrant pain, sometimes with hypotension and confusion as well. Here you must grade severity by the Tokyo criteria, understand that biliary drainage is the life-saving treatment, and choose the timing by grade: immediately after resuscitation in organ failure, early in moderate disease, and only when there is no response to antibiotics in mild disease. Candidates are also asked what to do when ERCP fails, and why the gallbladder is removed during the same admission after the duct has been cleared.

High risk of a duct stoneDuct stone on ultrasound, or cholangitis, or bilirubin above 4 mg/dL with a duct dilated above 6 mmERCP before cholecystectomy, or duct exploration during surgery
Intermediate riskAbnormal liver tests, age above 55, or a dilated duct on ultrasoundEUS or MRCP before surgery, or intra-operative cholangiography
Low riskNone of the findingsCholecystectomy without further work-up
Cholangitis grade I (mild)No organ failure and no grade II criteriaAntibiotics; drainage if no response within 24 hours
Cholangitis grade II (moderate)Two of: white cells above 12,000 or below 4,000, fever 39 or higher, age 75 or older, bilirubin 5 mg/dL or more, low albuminAntibiotics and early biliary drainage
Cholangitis grade III (severe)Organ failure: hypotension needing vasopressors, impaired consciousness, respiratory, renal or coagulation failure, or low plateletsResuscitation, antibiotics and drainage as soon as possible
