In most patients with PRIMARY (AL) AMYLOIDOSIS the marrow shows a MONOCLONAL GAMMOPATHY WITHOUT OVERT MYELOMA - a small clonal plasma cell population (often under 10%) with a serum or urine free light chain excess, demonstrable by immunofixation and free light chain assay. Overt myeloma and lymphoplasmacytic lymphoma account for a minority, and polyclonal plasmacytosis is reactive.
