In assessing a lung carcinoid one must COUNT MITOSES AND LOOK FOR NECROSIS: a typical carcinoid has fewer than 2 mitoses per 2 mm2 and no necrosis, an atypical carcinoid 2-10 mitoses or focal necrosis, and above that the tumour is a large cell neuroendocrine carcinoma or small cell carcinoma. Carcinoids DO contain dense core neurosecretory granules, and typical carcinoids have uniform round nuclei with 'salt and pepper' chromatin and ample cytoplasm - a high nuclear:cytoplasmic ratio with irregular nuclei suggesting small cell carcinoma.
