Recessive (autosomal recessive) optic atrophy is the most severe hereditary optic neuropathy in terms of both age of onset and depth of visual loss: it is present in infancy or the first years of life with nystagmus and acuity often worse than 6/60. Dominant optic atrophy begins insidiously in the first decade with milder loss, and Leber hereditary optic neuropathy presents in young adulthood.
