A unilateral acute angle closure with pressure of 50 mmHg and a shallow chamber, while the fellow eye has a deep chamber and a wide-open angle, points to a secondary, lens-induced mechanism - phacomorphic glaucoma from an intumescent cataractous lens pushing the iris forwards. In primary angle closure the fellow eye is anatomically similar and has an occludable angle, topiramate causes bilateral ciliochoroidal effusion with myopic shift, and the ICE syndrome is unilateral but chronic and associated with corneal and iris changes rather than acute closure.
