Epistaxis from a nasopharyngeal carcinoma that persists after posterior packing, in a haemodynamically stable patient, is managed by ANGIOGRAPHY WITH EMBOLISATION, which identifies and occludes the feeding branches (usually from the internal maxillary artery) and controls bleeding that tumour friability makes impossible to cauterise. Emergency resection is not the treatment for nasopharyngeal carcinoma (which is treated with chemoradiation), and injecting a coagulant endoscopically into a bleeding tumour bed is ineffective; a carotid blow-out or pseudoaneurysm after radiation must also be considered and is likewise treated endovascularly.
