In the Jahrsdoerfer 10-point grading scale for congenital aural atresia, the presence of the STAPES carries TWO points - the highest single weighting - and is the most important single predictor of a successful hearing result, with the other items (malleus, incudostapedial connection, oval window, round window, middle ear space, facial nerve course, mastoid pneumatisation and the appearance of the external ear) scoring one point each. A score of 8 or more indicates a good candidate for atresiaplasty; lower scores are better served by a bone conduction device.
