Congenital macroglossia is a feature of Beckwith-Wiedemann syndrome (with omphalocoele, hemihypertrophy, neonatal hypoglycaemia and a risk of Wilms tumour and hepatoblastoma requiring tumour surveillance); it also occurs in Down syndrome, hypothyroidism, mucopolysaccharidoses and lymphatic or vascular malformations. In Pierre Robin sequence the tongue is normal in size but retropositioned (glossoptosis) behind a small mandible.
