What distinguishes CHOLINERGIC from ADRENERGIC urticaria is the CLINICAL APPEARANCE: cholinergic urticaria produces tiny 1-3 mm punctate weals surrounded by LARGE ERYTHEMATOUS FLARES after heat, exercise or emotion, while adrenergic urticaria produces small papules surrounded by a WHITE HALO of vasoconstriction, provoked by stress and reproducible by intradermal noradrenaline. Both appear within minutes of the trigger and may be accompanied by systemic symptoms, and complement C4 is relevant to hereditary angio-oedema rather than to either.
