A TUBERCULOUS GUMMA (metastatic tuberculous abscess) arises by HAEMATOGENOUS dissemination, usually in an immunosuppressed or malnourished host, producing subcutaneous nodules that break down into undermined ulcers - as do acute miliary tuberculosis of the skin and some cases of lupus vulgaris. Tuberculosis verrucosa cutis follows exogenous reinoculation in a sensitised person, and scrofuloderma and orificial tuberculosis arise by CONTIGUOUS or autoinoculation spread from an underlying focus.
