Epinephrine should be avoided with blocks of end-artery areas (digital, penile, nasal or ear blocks and IV regional anesthesia) because of the risk of ischemia, although recent evidence suggests that dilute epinephrine in digital blocks is safe. Femoral nerve block has no contraindication to epinephrine, and epinephrine there reduces absorption and serves as an intravascular marker.
