PSMA PET in prostate cancer
PSMA is a membrane protein strongly expressed in prostate cancer cells, so PET with an agent that binds it (Ga-68 PSMA-11 or F-18-labelled agents) finds biochemical recurrence and metastatic disease at PSA levels far lower than CT and bone scintigraphy.
Why it matters in the exam
The exam asks mostly about biochemical recurrence: after radical prostatectomy — a rising PSA of 0.2 ng/mL or more; after radiotherapy — a rise of 2 ng/mL above the nadir (the Phoenix criterion). The detection rate of PSMA PET rises with PSA: about half of patients below 0.5 ng/mL and a large majority above 2 ng/mL. Under the EAU guidelines, PSMA PET is recommended in biochemical recurrence if the result will change treatment, and in primary staging of high-risk disease, where it is more accurate than CT and bone scintigraphy combined.
Pitfalls: physiological uptake in the salivary and lacrimal glands, kidneys, bowel, liver and spleen; uptake in sympathetic ganglia (coeliac, cervical) that looks like a lymph node; Paget disease, fractures and osteitis; and other tumours with new vessels that express PSMA (renal cancer, HCC, glioma). F-18 PSMA-1007 is excreted less in urine, so it is good for the pelvis, but causes more non-specific bone foci. About 5%–10% of tumours do not express PSMA, especially with neuroendocrine differentiation — there FDG helps.
Hormonal therapy (ADT) may temporarily raise PSMA expression in the first weeks and later lower it, so study timing affects interpretation. PSMA PET is also the condition for selecting patients for Lu-177 PSMA therapy. Older agents such as F-18 fluciclovine and C-11 or F-18 choline are less sensitive at low PSA. Structured reporting (PROMISE, E-PSMA) describes local disease, pelvic nodes, distant nodes, bone and organs.
Key points
- PSMA PET finds recurrence at low PSA.
- Recurrence after prostatectomy — PSA 0.2 ng/mL or more; after radiotherapy — 2 above nadir.
- Also recommended in high-risk primary staging.
- Pitfalls: ganglia, Paget disease, other tumours.
- PSMA-negative with high PSA — FDG.
Summary table
| Situation | Indication | Note |
|---|---|---|
| Primary staging at high risk | PSMA PET recommended | More accurate than CT and bone scintigraphy |
| Recurrence after prostatectomy | PSA 0.2 ng/mL or more and rising | Detection rises with PSA |
| Recurrence after radiotherapy | PSA 2 ng/mL above nadir | Phoenix |
| Before Lu-177 PSMA | Patient selection | Sufficient lesion uptake |
| Suspected PSMA-negative tumour | FDG PET | Neuroendocrine differentiation |

(the question as examined, in Hebrew)
