Epilepsy in women and pregnancy
In a woman with epilepsy the goal is a seizure-free pregnancy with minimal exposure to a harmful drug: plan ahead, replace valproate, give folic acid, and follow drug levels, which fall in pregnancy.
Why it matters in the exam
Questions present a young woman with epilepsy who wants to conceive, or who has found out she is pregnant, and ask what to change. The answers tested: valproate is avoided because of neural tube defects and developmental harm, lamotrigine and levetiracetam are the safe choices, treatment must never be stopped abruptly, and the lamotrigine level falls in pregnancy and rises after delivery. They also ask about contraception that enzyme inducers do not affect, and about telling an epileptic seizure from eclampsia.
Key points
- Valproate is avoided in women of childbearing age because of malformations and developmental harm.
- Lamotrigine and levetiracetam are the safest choices in pregnancy.
- Folic acid at least 0.4 mg daily, starting before pregnancy.
- Lamotrigine levels fall in pregnancy and rise after delivery — measure and adjust.
- Enzyme inducers weaken the pill; an intrauterine device is preferred.
Summary table
| Topic | Recommendation | Reason |
|---|---|---|
| Valproate | Avoid in women of childbearing age when an alternative exists; if none — only with a pregnancy prevention plan | The highest risk of malformations (mainly neural tube), lower IQ and autism |
| Preferred drugs | Lamotrigine and levetiracetam | The lowest malformation risk among the studied drugs |
| Folic acid | At least 0.4 mg daily, starting before pregnancy | Reduces neural tube defects and may improve development |
| Drug levels | Lamotrigine and levetiracetam levels fall in pregnancy; measure levels and adjust the dose, then reduce after delivery | Seizures in pregnancy endanger mother and fetus |
| Contraception | Enzyme inducers (carbamazepine, phenytoin, oxcarbazepine) reduce pill efficacy; an intrauterine device is the choice | Oestrogen-containing pills lower the lamotrigine level |
| Breastfeeding and delivery | Breastfeeding is recommended with most drugs; vaginal delivery is possible | A seizure in late pregnancy with hypertension — eclampsia, magnesium |
