Anatomy of the cavernous sinus and parasellar region

The cavernous sinus has two free "residents" — the internal carotid artery and nerve 6 lying lateral to it — and in the lateral wall, from top to bottom, nerves 3 and 4, V1 and V2; V3 is not in the sinus but leaves through the foramen ovale.

Why it matters in the exam

The Stage A neurosurgery exam shows cadaver dissection images through a pterional or endoscopic approach and asks what the arrow points to. So one must remember the order in the lateral wall, the position of nerve 6 inside the sinus itself — which is why it is the first injured by a cavernous aneurysm or thrombosis — and the Bouthillier segments of the carotid artery: C1 cervical, C2 petrous, C3 lacerum, C4 cavernous, C5 clinoid, C6 ophthalmic and C7 communicating. The clinoid segment lies between two dural rings: the proximal (lower) ring, formed by the carotid-oculomotor membrane under the anterior clinoid process, and the distal (upper) ring, where the artery becomes intradural. Removal of the anterior clinoid process exposes the clinoid segment and the optic nerve.

The sinus triangles serve as surgical windows: Parkinson's triangle between nerve 4 and V1 exposes the cavernous segment and the meningohypophyseal trunk; the anteromedial triangle between V1 and V2; the anterolateral triangle between V2 and V3, through which the sphenoid sinus and pterygopalatine space are reached; and the oculomotor triangle in the roof of the sinus, between the anterior and posterior petroclinoid folds and the interclinoid fold — where nerve 3 enters. The sinus drains to the superior and inferior petrosal sinuses and receives blood from the superior ophthalmic vein and the sphenoparietal sinus, so facial infection can reach it. In the endonasal approach the lateral opticocarotid recess is identified between the optic nerve and the artery, corresponding to the optic strut.
