The meandering mesenteric artery (arc of Riolan) is the enlarged collateral connecting the middle colic branch of the SMA with the left colic branch of the IMA; when the SMA is occluded, flow runs retrograde from the IMA through it to supply the small bowel, and its presence on imaging is a sign of chronic SMA occlusion. It is for that reason that the IMA must not be sacrificed in such a patient — so option B inverts the physiology.
