For acute irritable (dysphoric) mania, valproate is a first-line mood stabilizer, with rapid loading and good efficacy in mixed and irritable presentations. It is preferred to lithium (which is better for classic euphoric mania), and lamotrigine is ineffective in acute mania. After hysterectomy there is no pregnancy-related contraindication.
