Large cells with pale cytoplasm and prominent nucleoli scattered singly and in nests through the vulval epidermis, CK7- AND MUCICARMINE-POSITIVE AND S100-NEGATIVE, are EXTRAMAMMARY PAGET DISEASE of the vulva - an intraepithelial adenocarcinoma presenting as a chronic itchy erythematous plaque resistant to steroids, requiring wide excision and a search for an underlying anogenital or urothelial carcinoma in a minority. Melanoma would be S100 and SOX10 positive, HGSIL CK7-negative squamous dysplasia, and Langerhans cell histiocytosis CD1a-positive.
