Positive predictive value depends on prevalence as well as on sensitivity and specificity, so if prevalence falls while the test characteristics stay the same, the PPV falls substantially - more of the positives become false positives - while the negative predictive value rises. This is the central reason why population-wide screening for a relatively uncommon disease such as glaucoma generates many false positives, and why screening is targeted at higher-prevalence groups.
