Injury to the inferior oblique during lower lid blepharoplasty occurs when the dissection between the MEDIAL and CENTRAL fat pads is careless - the muscle runs between those two compartments - not between the central and lateral pads, so answer ב as written is not the mechanism. Cicatricial ectropion or lid retraction from excessive skin removal, orbital haemorrhage with an orbital compartment syndrome and visual loss from inadequate haemostasis, and diplopia from inadvertently incorporating the inferior oblique in a fat-repositioning suture are all genuine complications.
