In Duane retraction syndrome the lateral rectus is fibrotic and tight - histologically replaced in part by fibrous tissue and co-innervated by a branch of the third nerve because the abducens nucleus is absent or hypoplastic - so the surgeon encounters a tight, inelastic muscle with marked restriction on forced duction. This is why the operation is a recession procedure (often with a Y-split) rather than a resection, and why resecting the medial rectus would worsen the retraction and the globe narrowing.
