A diffuse infiltrate RICH IN PLASMA CELLS is characteristic of SYPHILIS at every stage, including the granulomatous infiltrates of TERTIARY disease (and should always prompt treponemal immunostaining and serology) - plasma cells also being prominent in borreliosis, rhinoscleroma, syphilitic and other chronic infections, in Zoon balanitis and in cutaneous plasmacytosis. Histoplasmosis shows intracellular yeasts in histiocytes, anaplastic large cell lymphoma sheets of CD30-positive large cells, and bowel bypass syndrome a neutrophilic infiltrate.
